All information is to be able to identify you within the lab/and your specimen test with the ordering Physician, or partner/affiliate with specialty, and genetic testing.  With your consent  to share information for job related wellness to return to work or for the purpose to continue to work due to state, government, county regulated laws. Core Lab Essentials will collect specific consents from you on https://www.corelabessentials.com during the test activation process, and will provide your genetic results to you.  Core Lab Essentials maintains your genetic data in compliance with HIPAA.
