Terms and Conditions

1. Acceptance of Terms For Service:

By accessing and using the services of Core Lab Essentials LLC. ("Company"), you agree to comply with and be bound by the following terms and conditions.

2. Services Provided

services, including but not limited to blood draws, specimen collection, and related medical services. All services are provided by licensed and certified phlebotomists.

3. Patient Responsibilities

Patients are responsible for providing accurate medical information and complying with pre-draw instructions provided by the Company. The Company (Core Lab Essentials LLC) is not liable for any adverse outcomes resulting from inaccurate information or non-compliance.

4. Privacy and Confidentiality

The Company is committed to protecting patient privacy and maintaining confidentiality in accordance with HIPAA regulations. Patient information will only be used for the purpose of providing medical services and will not be disclosed to third parties without explicit consent, except as required by law.

5. Liability

The Company (Core Lab Essentials LLC) and its employees shall not be held liable for any direct, indirect, incidental, or consequential damages arising out of or related to the use of its services. This includes, but is not limited to, injury, loss, or damage to property.

6. Payment Terms

Payment for services must be made at the time of service unless prior arrangements have been made. Partnered Labs must be done in office. Payment is at no charge unless it has been stated that it is split-payment. The Company reserves the right to refuse service for non-payment. All fees are non-refundable.

7. Cancellation and Rescheduling

Patients must provide at least 24 hours' notice for cancellations or rescheduling of appointments. Failure to do so may result in a cancellation fee.

8. Compliance with Laws

The Company operates in compliance with all applicable federal, state, and local laws and regulations. Patients agree to abide by these laws when using the Company's services.

9. Amendments

The Company reserves the right to amend these terms and conditions, and service prices at any time without prior notice. Continued use of the services after any changes constitutes acceptance of the new terms.

10. Governing Law

These terms and conditions are governed by the laws of the state in which the Company (Core Lab Essentials LLC) operates. Any disputes arising out of or related to these terms shall be resolved in the courts of that state.
